What is the difference between the two card options?
The Person Living with Dementia Card is designed to be carried and presented by someone living with dementia, memory loss, or another cognitive disability. It explains that they may need extra time, become confused, or have difficulty finding words. It also includes space to write their name and emergency contact information.
The Caregiver Companion Card is designed for caregivers and care partners. It explains that the person you're with is living with dementia and may need additional patience, reassurance, or support.
How many cards are included?
Each pack includes12 professionally printed cards.
What size are the cards?
The cards are credit card size (3.37" × 2.125"), making them easy to carry in a wallet, purse, pocket, travel bag, or badge holder.
They're designed to fit comfortably in standard wallet card slots, so they're always available when you need them.
What information can I write on the Person Living with Dementia Card?
The back of the card includes space to write:
- Name
- Emergency contact name
- Emergency contact phone number

Which card should I choose?
Choose the Person Living with Dementia Card if the individual living with dementia will be carrying and presenting the card themselves.
Choose the Caregiver Companion Card if you are a caregiver, spouse, family member, or care partner who would like a simple way to explain the situation to others.
What colors are available?
Both card styles are available in Navy Blue and Royal Purple.
Who created these cards?
These cards were created by a caregiver after supporting a father living with frontotemporal dementia (FTD) for more than 15 years. They were designed to make everyday interactions easier, less stressful, and more compassionate for everyone involved.
